As we approach the February 15th deadline for enrollment, we wanted to make sure you know of a few big enrollment events that are happening.
The first event is at the Resilience Center on Valentine's Day, February 14th, 2015 from 10:00 AM to 2:00 PM
The last event is held on February 15th, 2015 11 AM to 3 PM by Servicios de la Raza at 3131 W. 14th Ave
All events are on a first come, first serve basis. Please bring appropriate documentation to these events so that we can assist you in a more efficient manner.
Documents we may need:
Social Security numbers for everyone on application/household
Income information (W-2, paycheck stubs)
Photo ID (Driver's license)
Naturalization, Legal Residency Cards
Employer information (name, address, phone, insurance info)
Case number for public assistance
Medicaid denial letters
All assistance is free. Please be mindful of the deadline to enroll in any health plan for 2015 is February 15th. After this date, only individuals with certain qualifying life events are eligible to enroll in a health plan.
